CoolSculpting® is the brand name of a medical device used for non-invasive fat reduction. The underlying, scientifically recognised procedure is called Cryolipolysis (derived from the Ancient Greek „kryos“ = cold, „lipa“ = fat, „lysis“ = dissolution).

In cryolipolysis, fat cells (adipocytes) beneath the skin are damaged to such an extent by controlled, targeted cooling that they undergo programmed cell death (apoptosis), triggered by the cold, and are subsequently broken down naturally by the body via the lymphatic system.

The science behind it: How cold destroys fat cells

The discovery of cryolipolysis is based on the observation that fat cells are more sensitive to cold than the surrounding tissue, such as the skin, nerves and muscles.

  1. Targeted cooling An applicator cools the fat deposit to be treated to a temperature of approximately +4 to -11 degrees Celsius, depending on the applicator and the method used.
  2. Crystallisation & Cell Death At this temperature, the lipids within the fat cells begin to crystallise. This process damages the cell so severely that it undergoes programmed cell death (apoptosis).
  3. Natural degradation In the weeks and months that follow, the fat cells destroyed in this way are recognised by the body’s own scavenger cells (macrophages) and broken down via the lymphatic system and metabolism, before being processed by the body’s own cells.

What does a CoolSculpting treatment involve?

  1. Advice & Registration First, a detailed consultation is held to assess whether the patient is a suitable candidate. The areas to be treated are precisely defined and marked out.
  2. Skin protection A special gel pad is placed on the skin to protect it from frostbite.
  3. Fitting the applicator The appropriate applicator is fitted. It creates a vacuum that draws in the pocket of fat and pulls it between two cooling panels.
  4. Cooling phase The cooling phase begins and lasts between 35 and 75 minutes, depending on the treatment area and the applicator. At the start, you will feel a strong pulling sensation and intense cold, but these usually subside after a few minutes as the cold numbs the skin.
  5. Massage Once the applicator has been removed, the treated tissue feels hard and frozen. A subsequent two-minute, vigorous massage is crucial to aid the destruction of fat cells and accelerate their natural breakdown.

What is PAH? In the case of PAH, cold treatment does not lead to degradation, but rather to a Expansion and densification of the fatty tissue in the treated area. So, rather than becoming slimmer, the area actually becomes larger, firmer and often takes on the exact shape of the applicator used.

How common is PAH? The figures on prevalence vary. It was previously thought to be 1 in 20,000 cases, but more recent estimates suggest a higher incidence of up to 1 in 138.

Treatment: A PAH does not resolve on its own. The only effective treatment is usually surgery Liposuction (liposuction) of the hardened tissue, which is often more challenging than standard liposuction.

Not suitable is a treatment for general weight loss in cases of obesity or in people with certain cold-related disorders (e.g. cryoglobulinaemia).

How much does a CoolSculpting treatment cost?

The cost depends on the number of areas treated and the size of the applicators used. Charges are calculated per „cycle“ (one application of an applicator).

  • Small areas (e.g. double chin): approx. €450 – €800 per cycle.
  • Larger areas (e.g. stomach, hips): approx. €700 – €1,500 per cycle.
    Two cycles are always required to achieve a symmetrical result (e.g. on both hips). Several sessions are often recommended to achieve the best possible result.
