Buttock Contouring: An Overview of Modern Options

Buttock contouring is a term used to describe various plastic surgery procedures designed to improve the shape, size or firmness of the buttocks.

For many people, a shapely bottom is an important part of an aesthetically pleasing body silhouette. However, individual preferences and anatomical conditions can vary greatly. The term ‘buttock contouring’ therefore encompasses a whole range of plastic surgery procedures aimed at improving the shape, size or firmness of the buttocks.

This article provides you with a clear and structured overview of the various goals of modern bum-shaping and the methods best suited to achieving them.

What is bottom sculpting?

What is bum shaping?

Buttock contouring is an umbrella term for various surgical procedures in plastic and aesthetic surgery aimed at altering the shape, size, firmness or contour of the buttocks (gluteal region).

Depending on the desired result, various techniques are used, such as volume augmentation, skin tightening (lifting) or simply improving the contours through liposuction.

The aims of bum-shaping: What result are you hoping for?

To find the right method, it is crucial to define the primary treatment objective. In buttock surgery, a distinction is generally made between three main goals.

Aesthetic aimPrimary method(s)Principle of operationIdeal for…
More volume & projectionButtock augmentation (BBL using autologous fat or buttock implants)Volume augmentation through the injection of the patient’s own fat or the insertion of silicone implants.Patients who wish to achieve a significant increase in the size and roundness of their buttocks.
Firmer skin & liftingButtock lift (buttocks lift)Removal of excess skin to lift and tighten the remaining tissue.Patients who have experienced significant weight loss or who have sagging skin due to age or genetic factors.
Improved contours and silhouetteContouring liposuctionTargeted liposuction on areas around the bottom (e.g. flanks, lower back) to visually enhance it.Patients whose buttock shape is affected by surrounding fat deposits.

Objective 1: Greater volume and projection (augmentation)

If the buttocks are perceived as being too flat or too small, the aim is to enlarge them (also known as buttock augmentation). There are two established methods for this, both of which aim to add volume.

  • Enlargement using autologous fat (Brazilian Butt Lift – BBL surgery): In this procedure, the body’s own fat is extracted from another area, processed and injected specifically into the buttocks to shape and enlarge them.
  • Breast augmentation using implants: Firm silicone implants are placed either within the gluteal muscle (intramuscularly) or above the muscle, beneath the fascia (subfascially), to achieve a distinct, rounded projection and a significantly greater volume.

Objective 2: Firmer skin and lifting

Following significant weight loss, as a result of the natural ageing process or genetic predisposition, the skin on the buttocks may become loose and sag. The volume may be sufficient, but the shape is no longer firm.

  • Buttock lift (buttock lift): This procedure is primarily aimed at removing excess, loose skin. The surgeon makes incisions, usually in the upper buttock area, along the upper horizontal part of the buttocks (above the buttocks, near the lower back), in order to lift the tissue and restore a firm, youthful shape to the buttocks.

Objective 3: Improved contours and silhouette

Sometimes the volume and firmness of the buttocks are satisfactory in themselves, but the surrounding areas detract from the harmonious silhouette.

  • Body contouring through liposuction: Targeted liposuction on the flanks („love handles“), the lower back or beneath the buttocks (the so-called „banana fold“) can make the buttocks appear visually rounder, fuller and more defined – however, in some cases, a cautious approach to treating the banana fold is advisable, as excessive fat removal in this area can cause dimpling or sagging. This is often an integral part of a Brazilian butt lift.

The crucial role of expertise and risk mitigation

All methods of buttock reshaping are complex surgical procedures that require a high level of experience and aesthetic judgement on the part of the surgeon. Depending on the procedure, the risks can be significant, ranging from poor wound healing and asymmetry to serious complications such as a fat embolism following a BBL.

As these are cosmetic procedures, the costs of treating any complications or of any necessary revision surgery are not covered by health insurance. A Consequential Costs Insurance, which is taken out prior to the procedure, is therefore an essential safeguard against these high financial risks.

Frequently Asked Questions (FAQ) on buttock contouring

There is no single „best“ method. The choice depends entirely on your anatomy and your goals: if you want to add volume, you’ll need an augmentation. If you’re bothered by sagging skin, you’ll need a lift. An experienced specialist will often suggest a combination of procedures.

Building muscle through targeted exercise can significantly improve the shape of the buttocks and always provides a good foundation. However, a significant increase in volume in very slim individuals, or the removal of excess skin, is only possible through surgery.

The most important step by far is to have a detailed consultation with a qualified specialist in plastic and cosmetic surgery who has proven experience in buttock surgery. They will be able to assess your wishes against the anatomical possibilities and draw up a realistic treatment plan.
