Although the material does not usually leave the body, it can shift to the area surrounding its original injection site or appear to accumulate there, thus leading to aesthetically undesirable bulges.

Contact us for a no-obligation enquiry

How can I tell if hyaluronic acid has shifted?

It is important to distinguish this from normal post-operative swelling (which subsides after a few days with proper aftercare). Filler migration usually only becomes apparent weeks or months after treatment, although it can also become visible sooner.

Typical signs in the most common problem areas are:

On the lips (most common case): A visible, often whitish, shimmering ridge forms above the actual red part of the lips (the lip contour). The lips appear unnaturally pushed forwards („duck bill“ or „duck lips“). The upper lip loses its natural shape.

Around the eyes (tear duct): Possible permanent or long-lasting, bluish-tinged swellings, which may indicate that hyaluronic acid has been injected too superficially (often in connection with the Tyndall effect) or that the material has shifted.

Cheeks/jawline: Palpable lumps or asymmetrical swellings that can be moved.

The 4 main causes: Why does the filler migrate?

The causes of migration are varied and usually stem from a combination of medical technology and anatomy.

1. Too much material (overfilling): The skin and tissue (particularly on the lips) have only a limited capacity. The volume and the resulting swelling can overload the tissue, causing the filler to be pushed or displaced into neighbouring areas, either visually or physically.

2. Incorrect injection site: If the filler is injected too shallowly into the skin or the wrong technique is used, it cannot anchor itself in the tissue and is more likely to shift.

3. Wrong product: Hyaluronic acid fillers with different viscosities and consistencies are designed for specific areas. A filler that is too soft in dynamic areas such as the lips is more likely to shift or move out of place.

4. Intense muscle activity (facial expressions): The circular muscle around the mouth is constantly in motion (talking, eating, kissing). This constant mechanical strain can cause the material to be pushed out of the lip over a period of months.

Your role following treatment: Failure to take care in the first 48 hours after the injection (exerting strong pressure, massaging the injection sites, exercising) can also increase the likelihood of migration. Please follow your doctor’s aftercare instructions strictly.

Contact us for a no-obligation enquiry

The solution: Hylase – not a miracle cure

If hyaluronic acid has migrated, many doctors recommend dissolving the substance. The standard procedure for this is the injection of the enzyme hyaluronidase (Hylase). This enzyme breaks down the molecular chains of the hyaluronic acid, enabling the body to eliminate it more quickly via the lymphatic system.

Hylase is often portrayed as a simple „miracle cure“ that can reverse any filler-related complication in a matter of seconds. The medical reality, however, is more complex. A Hylase treatment requires the highest level of expertise and carries its own risks, which should not be underestimated:

Stubborn filler residue

Modern hyaluronic acid fillers are extremely highly cross-linked to ensure they remain in the body for a long time. These firm structures are often difficult for Hylase to break down. Several painful sessions, spaced several weeks apart, are usually required. Complete removal of the migrated material is not always guaranteed.

Breakdown of the body’s own tissue

Hylase does not distinguish between the artificial filler and your body’s own natural hyaluronic acid. The enzyme inevitably breaks down the natural supportive structure of your skin in the area where the injection was administered.

The result: this can lead to a temporary „deflation“ effect (e.g. skin that appears dull, dry and slightly sunken), which usually returns to normal within a matter of weeks or months.

Allergic reactions

Hyaluronidase is a highly potent enzyme. Although rare, there is a genuine risk of severe allergic reactions (including anaphylactic shock). Many doctors routinely carry out a preliminary allergy test (patch test) on the forearm.

Unpredictable diffusion

As Hylase is an aqueous solution, it often spreads uncontrollably within the tissue. It is extremely difficult to dissolve only the „incorrect“ filler lump without also destroying the desired volume in the correct place.

The bitter truth about permanent fillers

Enzymatic dissolution using hyaluronidase is currently only possible with hyaluronic acid fillers. For permanent fillers, biostimulators such as Radiesse or Sculptra, and autologous fat, there are generally no comparable dissolution methods available.

Financial protection in the event of complications

Filler migration is a medical complication whose diagnosis (e.g. using high-resolution ultrasound) and correction (multiple Hylase treatments or even surgical removal) can incur significant additional costs. As these treatments follow a primarily aesthetic procedure, statutory and private health insurance schemes do not usually cover these costs (in accordance with Section 52(2) of the German Social Code, Book V (SGB V)).

Correcting a migration is often considerably more expensive and more stressful than the original injection. For such unforeseeable circumstances, it may be advisable to take out Consequential Costs Insurance provide essential financial security.

Contact us for a no-obligation enquiry

Frequently Asked Questions (FAQ) on filler migration

Is filler migration dangerous?+

In the vast majority of cases, filler migration is primarily an aesthetic problem, but is medically harmless. Modern MRI and ultrasound data show that residual material in certain areas (e.g. the tear trough, lower cheeks) can persist for several years; however, complete and reliable resorption after 12 months is the norm for most commonly used preparations. Simply waiting is usually frustrating; active dissolution by a specialist is often unavoidable.

When can a new hyaluronic acid injection be administered after the previous one has been dissolved?+

Patience is key here. Following a Hylase treatment, the tissue must heal completely and the body’s own hyaluronic acid must regenerate. A follow-up treatment should be carried out after 4 to 6 weeks at the earliest, or preferably after around 3 months.
