The capsule is present but causes no symptoms and is not visible from the outside. No action is required.

Grade II Baker’s disease is palpable but not visible. Conservative measures — regular massage, ultrasound treatments and vitamin E supplementation — can keep the condition stable at this stage in the long term. A follow-up medical check-up after 6 months is advisable.

Baker Grades III and IV: When is surgery unavoidable?

Baker Grade III means: The capsule is visibly deformed; the breast feels firm and looks unnatural. This is accompanied by pain on movement or when pressure is applied. In such cases, the DGPRÄC guidelines recommend active correction.

Grade IV Baker is the most severe form of capsular contracture. The capsule is rock-hard. The breast is severely deformed and causes constant pain. Surgery is urgently required — any further delay will compromise the outcome of the operation.

Important: From Baker Grade III onwards, 4beauty’s follow-up costs insurance covers the replacement of implants, including the cost of the implants — up to €10,000.

Treatment methods for capsular fibrosis: from conservative to surgical

Key points at a glance

Conservative treatment: only recommended for grades I–II (massage, ultrasound, vitamin E)

Capsulotomy (fibrosis incision): gentle, but with an increased risk of recurrence

Capsulotomy + implant replacement: the gold standard for Grade III and IV

Explantation: the safest long-term solution in the event of recurrence or intolerance

Cost: €1,500–8,000 depending on the method; statutory health insurance rarely covers the cost

Treatment for implant capsular fibrosis is based on the Baker grade and the patient’s individual anatomy. Not every method is suitable for every degree of severity.

Capsulotomy: Fibrosection as a minimally invasive option

Capsulotomy (fibrosis incision) involves making an incision in the hardened capsule without removing it completely. The implant remains in place. The procedure takes less time than a capsulectomy and has a shorter recovery time.

Suitable for: Baker Grade III with moderate symptoms. The risk of recurrence is higher than with complete capsulotomy — the capsule may reform. For this reason, capsulotomy is now less frequently recommended as a stand-alone procedure.

Capsulectomy: Complete removal of the capsule

Capsulectomy involves the removal of the capsular tissue either completely (total capsulectomy) or partially (subtotal capsulectomy). In cases of Baker grade IV or following a recurrence after capsulotomy, total capsulectomy is considered the gold standard.

The procedure is often carried out in conjunction with an implant replacement. Textured implants or those with improved biocompatibility have been shown to reduce the recurrence of capsular contracture.

Implant replacement: a new implant, a better result

Implant replacement is the most common treatment for Baker Grade III and IV. The old implant is removed, the capsular tissue is resected either completely or partially, and a new implant is inserted.

Textured implant surfaces and submuscular placement (under the pectoral muscle) reduce the risk of recurrent capsular contracture. Smooth implants remain an alternative for patients who cannot tolerate textured models.

Method

Indication (Baker grade)

Budget

Risk of recurrence

Conservative treatment

Grade I, II

€200–600

moderate

Capsulotomy (fibrosis incision)

Grade III

€1,500–3,500

increased

Capsulotomy + implant replacement

Grade III, IV

€3,000–8,000

minor (in the event of a change in technology)

Implant removal + autologous tissue reconstruction

Grade IV, recurrence

€5,000–12,000

very low

The ‘no-touch’ method and modern implant technology for prevention

No-touch method (Keller Funnel): The implant does not come into contact with any skin surfaces — minimised risk of contamination

Submuscular placement: Placing the implant beneath the pectoral muscle reduces capsular contracture

Antibiotic prophylaxis: reduces the risk of infection before and during surgery

Textured implant surfaces: have been shown to carry a lower risk of capsular contracture than smooth surfaces

The No-touch method The Keller Funnel is a modern surgical technique in which the silicone implant is inserted via a sterile funnel. On top of this come the costs of anaesthesia, hospital fees and the cost of the new implant itself.

Anyone who has not budgeted for this amount faces a considerable financial burden.
