If you are unsure, or if you require a personalised treatment plan, it is essential that you consult a qualified specialist.

Enlarged male breasts can cause considerable self-consciousness. However, this is not always caused by a hormonal imbalance or an increase in glandular tissue, as is the case with true gynaecomastia. Often, it is a condition known as pseudogynaecomastia, also referred to as lipomastia. This glossary entry explains what pseudogynecomastia is, how it differs from gynecomastia, how it develops, how it is diagnosed, and what treatment options are available, including liposuction.

What is pseudogynecomastia (lipomastia)?

Pseudogynecomastia (from the Greek pseudo = false, gyne = woman, mastos = breast) refers to an enlargement of the male breast caused exclusively by an increase in fatty tissue in the chest area. Unlike „true“ gynaecomastia, pseudogynaecomastia does not involve any increase in mammary gland tissue. It is therefore purely a result of fat accumulation, which is why it is also referred to as lipomastia (fat breast) or adipomastia. This breast enlargement in men is usually harmless, but may be perceived as a cosmetic concern.

Difference: Pseudogynaecomastia vs. true gynaecomastia

It is crucial to distinguish between pseudogynecomastia and true gynaecomastia, as the causes of gynaecomastia and pseudogynecomastia, and the approaches to their treatment, differ fundamentally:

Pseudogynecomastia (lipomastia):

Cause: A simple build-up of fatty tissue.

Mammary gland / mammary gland tissue: No increase.

Texture: Soft, doughy.

Hormonal causes: These are not usually directly involved.

True gynaecomastia:

Cause: Proliferation of breast glandular tissue, often due to hormonal factors (a hormonal imbalance, e.g. caused by the conversion of hormones or reduced testosterone activity) or as a result of medication or medical conditions. It is sometimes a secondary symptom of other conditions.

Glandular tissue: Clearly palpable, often as a firm lump or cord behind the nipple.

Consistency: Firm, rubbery, sometimes tender or painful.

Types: It can occur on both sides or on one side only. A common form is pubertal gynaecomastia, which occurs during puberty and often resolves on its own. If it persists chronically or is severe, treatment may be considered. In rare cases, medicines such as tamoxifen or clomiphene may be used for treatment, but this should be managed by specialists in endocrinology.

In some cases, mixed forms – known as lipogynaecomastia – may also occur, in which there is an increase in both fat and glandular tissue.

Causes and triggers: What causes pseudogynaecomastia?

Overweight and obesity: In cases of severe overweight, a general increase in body fat percentage often leads to increased fat deposits in the chest area.

Lifestyle: A high-calorie diet and a lack of exercise contribute to weight gain and, consequently, the development of lipomastia.

Genetic predisposition: The way fat is distributed in the body can be genetically determined.

Less common causes: In rare cases, benign fatty tumours (lipomas) in the breast area, where excess fatty tissue accumulates, can also mimic pseudogynecomastia.

How can you tell if a man has pseudogynecomastia?

Soft consistency with no hardened glandular tissue.

Usually no pain or tightness.

It often occurs on both sides, but can also be unilateral or asymmetrical.

Many of those affected find the enlarged breast aesthetically unappealing and suffer from psychological distress that affects their body image. The main symptom of pseudogynecomastia is a visible and palpable enlargement of one or both male breasts.

Diagnosis: How does a doctor diagnose pseudogynecomastia?

A medical examination is essential in order to reliably diagnose pseudogynaecomastia and distinguish it from true gynaecomastia or other rare pathological breast conditions. The diagnostic process typically involves:

Medical history and consultation Taking a patient’s medical history.

Physical examination (palpation) Breast examination.

Ultrasound (sonography) Key examination for distinguishing between adipose and glandular tissue.

Further investigations (rare) Mammography or hormone tests. Treatment by liposuction is therefore primarily considered to be for cosmetic or aesthetic reasons. As a result, it is rarely covered by statutory (or private) health insurance.
